A tilted image no longer fills a rectangle, so its corners would be empty. By default the tool crops in just enough to remove them, keeping the original shape. You can turn that off to keep the whole picture; the corners are then transparent in PNG and WebP, or a colour you choose in JPG.
Rotating by a quarter turn or flipping never loses a pixel. Straightening redraws the image at an angle, which softens it very slightly.
Phones store which way up a photo was taken in a hidden tag rather than turning the pixels. Some sites and apps ignore the tag and show the photo on its side. This tool applies the tag when it opens the image and saves the pixels the right way up, so the downloaded file looks the same everywhere.
Flipping is also how to undo a selfie that came out mirrored, so text in the background reads the right way round.

Quarter turns and flips move pixels without changing them. Save as PNG for an exact copy, or as JPG or WebP at a high quality for a smaller file. Straightening at an angle redraws the image and softens it slightly.
